CRITERIA FOR CHILD WITH DISABILITIES

Yes   No   The child is 3, 4, or 5 years old and;

Yes   No   Experiences a severe delay in development.
 

EXCLUSIONARY FACTORS:

Delay in development is due to factors related to environment, economic
disadvantage, or cultural difference.
 

DEFINITIONS:

A severe delay in development means:

(a) the child functions at a developmental level two or more standard deviations
below the norm in any one area of development or 1.5 standard deviations
below the norm in two or more areas of development; and

(b) the areas of development include one or more of the following areas:
cognitive development, physical development, communication development,
social and emotional development, or adaptive functioning skills.
